Register with our practice
To register as a new patient you will need to complete this form.
Alternatively you can drop into the practice and pick up a form to complete and hand back in. Please do this after 11am, as these are our quieter times and any questions you may have can be answered without interruptions.
To complete your registration, you will be asked to provide a proof of ID. The receptionist will take a copy, and it will be kept on your records.
NB – Please bring along your medication when coming to the practice for the first time.

Accessing to Health Records and Proxy Access
The practice is registered and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. If you wish to request access to the records held by the practice, please contact our reception staff on 024 76614255.
